Health insurance cancellation letter template in Word (DOCX) to fill in
Prefer to type your details on screen before printing? An editable Word template takes just a few minutes to complete. Here is what it must contain, how to fill it in without forgetting anything, and how to send it on time.
Why choose a Word template over a PDF
A Word template (.doc or .docx) has a concrete advantage: you fill in the fields directly on screen — name, surname, address, insured or policy number, desired effective date — before printing a clean, legible version. It is faster than a blank page and avoids crossings-out. The DOCX format also stays editable if you need to fix a typo or adapt the letter to another contract.
PDF is useful for archiving or printing an identical copy. Legally, however, no format is required: a letter typed in Word, then printed and signed, has exactly the same value as a handwritten one. What counts is the content, the signature and meeting the deadline.
The fields to fill in the editable template
Whatever word processor you use, always complete the same items: your full identity (name, surname, address, date of birth), the insured or policy number shown on your card or policy, a clear statement of your intention to cancel, the line concerned (basic insurance LAMal and/or supplementary LCA) and the desired effective date. Date the letter to the actual day of writing.
For basic insurance you do not have to give a reason for leaving. Nor do you have to name your future fund for the cancellation to be valid — but because affiliation to the compulsory health insurance is continuous, have a new fund ready for 1 January.
Print, sign and send on time
Once the Word template is filled in, print it and sign it by hand if you send it by post. Keep a copy. For basic insurance, registered mail is strongly advised: it gives a certain date and proof of receipt, essential in the event of a dispute.
Watch the calendar. For an exit on 1 January, the cancellation of basic insurance must REACH the fund by 30 November at the latest (one-month notice). As receipt is decisive and not the postmark, do not wait until the last day. For supplementary LCA cover, the notice period is generally three months (cancellation by 30 September for an end on 31 December), subject to any minimum term set out in the general conditions.
★ Good to know
The file format (Word, DOCX, PDF) has no bearing on validity: only the correct details, the signature and receipt within the deadline matter. For basic insurance, the date the fund receives your letter is decisive, not the postmark.
