Free health insurance cancellation letter template, to fill in

Cancelling costs nothing: a simple letter is enough. Here is a free template, the fields to complete, and the pitfalls to avoid so it is accepted.

The fields to complete in a free template

A good free template clearly shows the areas to personalise. You first fill in your contact details (surname, first name, address, date of birth), then the insured or policy number as shown on your insurance card. Then comes the fund's address, to be taken from your policy or statements — not a local agency that does not handle cancellations. Finally, you state the desired effective date and sign.

For basic insurance there is no need to add a reason: an ordinary cancellation does not have to be justified. Stay factual and concise; a clear cancellation sentence is better than a long explanation.

Free does not mean sloppy

A free template remains legally valid provided it is complete. The most common mistake is forgetting the policy number: without it, the fund struggles to link the request to the right contract, especially when several household members are insured. So prepare a separate letter per person rather than a collective letter.

Also check the consistency of the dates: the date of writing is today's, while the effective date is 1 January for an ordinary cancellation of basic insurance. Once filled in, re-read the document before printing and signing it.

Can the fund charge me for the cancellation?
No, not for basic insurance. Cancelling is a right and incurs no fee. However, unpaid premiums or cost-sharing can block the exit until they are settled.
What happens if I forget a field?
A missing field, such as the policy number, can delay or jeopardise processing. Re-read the filled-in template before sending it and keep a signed copy.
Does the free template work for supplementary insurance too?
The principle is the same, but the deadlines differ: supplementary insurance (LCA/VVG) follows a 3-month notice period and any minimum terms set in the contract. Check your general conditions.
